By submitting this credit application, I am requesting Burbank Water & Power (BWP) to supply electrical service and/or water service to the service address submitted. I also promise to pay in accordance with the rate schedules of BWP of the City of Burbank. I further promise to comply with all applicable City ordinances, rules, and regulations. I represent that I am one of the owners, or I have the legal right of possession of, the address submitted and I hereby give the City of Burbank and its authorized representatives the right to access such service address property to provide the services requested. I waive any and all claims for damages which may from this date forward arise from the starting or discontinuing of any or all electrical services or water services at the listed service address. I further acknowledge that I have read and understand this application and that I will electronically receive a confirmation verification of receipt of this application which contains this agreement. I further represent that if I have submitted an application for a person or an entity other than myself, that I have the legal authority to bind such person or entity for any and all charges for municipal services provided to the service address property.
